In- Home Counseling Services
Does your child has a history of behavioral health challenges, or Do you believe that your child would benefit from behavioral health services, please call 1-877-652-7624 and request a “Needs Assessment” to be completed by: Darbouze, LLC- Provider# 0550035. You can also request therapeutic services from your Care Manager (CMO worker) or Mobile Response Worker (CMR worker) for a therapist or Behavioral Assistant from Darbouze, LLC.
Darbouze, LLC believes that the family is the most powerful social institution and we support our families to maintain the family unit.
Intensive In-Community (IIC) services are designated for children and adolescents throughout the state of New Jersey. Darbouze, LLC is an in-home provider through the New Jersey Children's System of Care.
The State of New Jersey offers families limited therapeutic services for children who experience emotional and behavioral difficulties. In order to obtain services for your child, you must
- Contact Performcare at 877-652-7624.
You can request Darbouze, LLC as your child’s in-home provider. The process may be long but, the benefit is that your child may receive in-home therapy from a licensed professional at no charge to you.
2. Describe the behavioral, mental, or emotional problems that your child is exhibiting to the Performcare Representative.
Remember to be as descriptive as possible with regards to the behavioral or emotional issues that your child is experiencing and/or exhibiting in the home, school, and or community.
As a parent/caregiver your initial instinct may be to minimize the concerns pertaining to your child's behaviors but in order to receive the services, you must be specific and honest with regards to your child's behaviors. You may also request that Darbouze, LLC- Provider# 0550035 provide services for your child.
3. Next, Performcare will assess if there is a need for some type of therapeutic intervention.
Types of intervention from Performcare
- If you select Darbouze, LLC as your provider, we will be notified, if it is determined that your child requires an evaluation called BPS/Needs Assessment.
Then, a Licensed Clinician from Darbouze, LLC will contact you to schedule a Psycho-social Evaluation/Strength and Needs assessment with you and your child. This evaluation is completed by the Licensed Clinician to determine whether or not in-home services are appropriate for your child.
After the Assessment is completed, the clinician from Darbouze, LLC will advise you of their recommendation.
- The Licensed Clinician will complete the evaluation/assessment and submit it to Performcare.
- You will then follow up with PerformCare at 1-877-652-7624.
- Please note that the Biopsychosocial Assessment does not guarantee a link to IIC or in-home services or case management services.
If Performcare approves services, you will receive a call from the care manager from your county's CMO office. Once you have contact with that case manager- you can request Darbouze, LLC- Provider# 0550035 as your therapeutic provider. Darbouze, LLC will contact you to schedule therapy sessions.
- The Performcare representative can also refer you to Children's Mobile Response (CMR) for immediate services and contact will be made within 24 hours of your call. If Mobile Response determines that your child qualifies for Services, you can request Darbouze, LLC- Provider# 0550035 as your provider and we will contact you within 24-48 hours.

We are offering in person and telehealth services for our Intensive In-Community (IIC) program for children under age 21 via Performcare.
